My Nurse is a free health information service for the public, sponsored by UnityPoint Health and staffed by registered nurses 24-hours a day, 7 days a week.

Billing and Insurance Information

Billing Information

• Cash, personal checks, VISA, MasterCard and Discover are accepted.
• Accounts not paid within a reasonable time period may be placed with a collection agency following due notice.
• Our clinic staff can assist you in finding mutually beneficial solutions. Options can include paying by credit card, making payment arrangements and applying for financial assistance.

Insurance Information

• We will file claims to all insurance companies.
• We participate with most major insurance companies.
• All co-payments are due at the time of service.
• If you are covered by an insurance we do not participate with or do not have insurance, a minimum payment of 20 percent of your total charge is required at the time of service.
