Health Insurance |
Employees may have coverage for vision, chiropractic, pharmacy, and mental health/substance abuse benefits.
Employee premiums are taken each pay period and are deducted on a pre-tax basis allowing employees to lower taxable income and increase take home pay. |

Prescription drug coverage through participating pharmacies. Co-payment for prescriptions are determined by: Generic: $10, Formulary Brand: $30, and Non-Formulary: $60. |

Paid Time Off (PTO) |
PTO combines vacation, holidays, sick and personal days into a flexible plan up to a maximum of 200 hours per year for non-exempt employees and 240 hours for exempt employees.
Accrual rates increase after five years of employment is completed. |

Short Term Disability (STD) |
This plan will cover a maximum of 26 weeks of disability and pay at 60% of the employee's average weekly wages.
Coverage begins on the 8th calendar day of being away from work due to a non-work related injury or illness. |
Any regular full-time, part-time or part-time accruing employee averaging at least 16 hours per pay period.
After the employee has worked 1,000 hours at Iowa Health. |

Life Insurance |
Core life for a regular full-time employee is 1 x base annual salary rounded up to nearest $1,000.
Core life for a regular part-time employee is $10,000.
Eligible employees are given the option to carry additional life insurance on themselves, spouses, and their dependents. |

Pre-tax retirement savings plan. Iowa Health-Des Moines will automatically contribute 2% of your pay into your account each year.
You may contribute up to $16,500 in 2009 and Iowa Health will match 50% of your contribution up to 6% of your pay. |
