Birth Certificates :: duplicate/replacement
In order to get a duplicate copy of your birth certificate, contact the Iowa Department of Vital Records at 1-515-281-4944. They charge approximately $10.00 per copy. Iowa Methodist, Methodist West and Iowa Lutheran do not provide copies of birth certificates.

Helping to Care for Our Diverse Community
Iowa Health - Des Moines provides language interpretive services to patients and families who have limited fluency with English, and also those who are deaf or hard of hearing. Spanish interpreters provide 7 a.m. to 7 p.m. weekday coverage at Methodist, Methodist West and Lutheran and on-call coverage on weekends. Many discharge instructions and legal documents are available in Spanish. In addition to Spanish, we strive to provide translation services for speakers of Arabic, Vietnamese, Russian and Laotian.

Our staff also uses Language Line, a 24/7 telephone interpretation service, and additional translation resources as necessary. We have many other events and resources to assist diverse populations in individual care areas and throughout our health system. These services are available to patients while in our facilities and can be accessed simply by requesting language assistance from any staff member.
