New Patient Information
In order to properly schedule, it is important to inform us of the reason for your visit.
When you are extremely ill, we will make every effort to see you the same day.
If other members of your family need to be seen, please schedule separate appointments.
Please notify us at least 24 hours in advance if you need to reschedule your appointment.
It is helpful if you arrive 10 minutes early for your appointment to allow time for the registration process. We request that you present your insurance card and photo ID at each visit to create a more efficient check-out and billing process. We make every effort to stay on schedule and you will be informed if the physician is delayed.
Treatment of Children
Minors not accompanied by their parent or guardian must have written consent for treatment or their guardian must call the clinic with verbal consent.
Medical Record Release
Your medical records are confidential and require written consent by the patient or guardian in order to be released from any clinic location. Processing requests for medical records could take up to two weeks. Please request all copies in advance so that we can meet your needs in a timely manner.
